Adult Day Care services is a community based program designed for adults who need assistance, are disabled or chronically ill but prefer, and are safe, to live at home. Sapphire Nursing and Rehab at Goshen’s Day Care programs are under the direction of a Registered Nurse and delivers professional care in collaboration with a personalized interdisciplinary team. Both social and medical model day cares are offered at Sapphire Nursing and Rehab at Goshen. Our nursing home team is comprised of skilled, caring, highly trained staff to meet the specific needs of each of our residents. Sapphire Nursing and Rehab at Goshen’s skilled nursing services, both short-term and long-term, provide a variety of medical, clinical, social and psychological components to address the needs of individuals when the level of care needed can no longer be provided at home.
