Pine Haven Christian Communities includes 4 campuses: Our Giddings Avenue Campus is located at 531 Giddings Avenue in Sheboygan Falls. This campus is the site where our original 12 room nursing home opened in 1950. It currently offers 86 rooms for assisted living, and 51 rooms for assisted living with memory care. Our Prairie Crossing Campus is located at 1280 Pine Haven Lane. This campus offers 40 apartments for supported independent living. The Haven Drive Campus houses skilled nursing, assisted living, mid to late stage memory care, and rehabilitation. There are two skilled nursing households for long term care (18 rooms each) and a separate rehabilitation household of 14 rooms, with a state-of-the-art therapy gym. There are 32 rooms of memory care - 24 for stage 2 and stage 3 memory care and 8 for those who have both memory care and high physical acuity needs. There are 8 traditional assisted living rooms at this campus. Our Oostburg campus is located at 701 Pine Drive in Oostburg, which provides 30 rooms of traditional assisted living. read more
