As one of the largest Family Health Teams in Ontario, the organization works with over 100 primary care physicians providing comprehensive care to their patients. Collectively, we have over 100 inter-professional healthcare providers, comprised of: mental health clinicians, nurse practitioners, pharmacists, a physician assistant, registered dietitians, registered nurses and registered practical nurses. In addition to one-on-one care, PFHT administers many programs, services, and workshops as a means to redirect patients from the providers practice to a more focused area of care. This approach increases capacity at the practice level while providing patients with an opportunity to learn how to manage their health. These programs, workshops, and services are offered at no charge and derive from irrefutable patterns displayed in the perpetual symptoms patients present. PFHT is a Queen’s residency site, allowing our primary care physicians to have the chance to teach new physicians.
