Our members are 41 GP practices that serve the local population and help us shape the direction of the CCG. We plan and pay for services like: - planned hospital care; - rehabilitation services (such as physiotherapy or occupational therapy); - most community services (eg district nurses); - urgent and emergency care,including A&E, ambulances and out-of-hours services; - mental health services; - maternity services; - continuing healthcare We do not buy primary care services from GP practices, pharmacies, dentists and opticians. This is done by NHS England. We bring together healthcare professionals, local people and managers so people have more control of their own care; to provide a greater focus on healthcare and quality; to increase the involvement doctors and nurses have in the care and services delivered; and to improve the health and experiences of local people. The change to a clinically-led NHS is a fantastic opportunity to focus on delivering improved health outcomes, quality, innovation and public engagement. We've started to see results from this new way of working, which will bring real benefits to patients; and we want to work better with local people as we improve services and health patients live longer, healthier lives. Following our vision - better health for the people of Bradford - in 2013/14 we have focused on avoiding preventable deaths caused by cardiovascular disease; helping people with long-term conditions to live better lives; and improving the patient experience. The number of people suffering a stroke has reduced by almost 15% over the year; the number of people receiving key tests for diabetes is now 60%; and the number of premature deaths - under 75 years old - has also reduced by 10%. read more
