The Mid Wales Healthcare Collaborative is responsible for delivering a single integrated change programme, with full public a ... nd professional participation, which addresses the delivery of social care, primary care and specialist care as an integrated continuum, provided as close to home as possible. It will address prevention as well as treatment, and promote prudent healthcare. The Collaborative will conduct its work in a way that reflects respect and understanding, open and honest engagement, and inclusiveness. The Mid Wales Healthcare Collaborative will work with providers and communities to plan and deliver safe, sustainable, high quality and accessible health and social care services for the population of Mid Wales. The Mid Wales Healthcare Collaborative will identify and prioritise work to achieving this in relation to: • Staff and services in Primary & Community Care, including Community Hospitals • Pathways for more advanced care to meet the needs of people of Mid Wales, ensuring accessible secondary care services • Facilities, protocols and pathways for specialist referrals to hospitals outside the region that respect practicalities, convenience and dignity of the users • Recruitment and retention of appropriately skilled staff across all services • Information about services available and about progress made by the Collaborative • Quality and performance outcome measures appropriate to the delivery of health care in a rural area • Academic research, training and professional skills development The members of the Mid Wales Healthcare Collaborative are Betsi Cadwalader University Health Board, Hywel Dda University Health Board, Powys Teaching Health Board and the Welsh Ambulance Service Trust.
