As a charitable organization, Lumina Hospice & Palliative Care is funded — in part — by the generous support of our local community. In turn, this enables us to provide the highest in quality care for our neighbors while exceeding the levels required by Medicare and private insurance. Our services: Hospice Care - A supportive approach to end-of-life care provided at the patient’s residence. Palliative Care Case Management - An interdisciplinary medical program for patients living with a serious or chronic illness who are not yet ready for hospice Palliative Care Consults - Clinical guidance for medical providers and patients to assist with symptom management and advance care planning Transitions - A non-medical community program to help individuals live well with a serious, life-limiting illness.
