HOMETOWN Health Center understands and incorporates an integrated approach to its health care treatment philosophy whereby the medical services are seamlessly blended with behavioral health services to provide complete and well-balanced care for the patient. HHC has a 24 hour/7 day per week FoneMed system which provides our patients with access to a nurse and provider who offer clinical advice when our offices are closed. This system is designed to best meet the needs of our patients, even in the middle of the night. The FoneMed system can be reached by calling any of our offices and following the automated prompts.
