Our services are 100% free of charge to clients as they are covered either by Medicaid or grant funding. We focus on tangible health outcomes such as reducing emergency room visits and inpatient hospital stays. We do this by providing personalized care management to assist in obtaining and maintaining comprehensive, preventative medical care (e.g. appointment scheduling, reminders and appointment escorts) as well as addressing psycho-social needs that might interfere with medical care.
